🧪 Overview

Matrix metalloproteinase-9 (MMP-9) degrades tight junction proteins (claudin-5, occludin, ZO-1) and extracellular matrix components of the neurovascular unit. The balance between MMP-9 and its inhibitor TIMP-1 determines the extent of BBB paracellular leakage. An elevated MMP-9/TIMP-1 ratio in CSF may serve as an early biomarker for neurodegeneration, but significant confounds from systemic inflammation and the invasive nature of CSF collection limit clinical utility.
