Cerebrospinal fluid (CSF) and blood biomarkers for 4-repeat (4R) tauopathies provide critical diagnostic and prognostic information for distinguishing corticobasal degeneration (CBD) and progressive supranuclear palsy (PSP) from other neurodegenerative disorders. 4R tauopathies are characterized by the predominant accumulation of tau isoforms containing four microtubule-binding repeats, in contrast to the mixture of 3R and 4R tau found in Alzheimer's disease["@bladowska2020"]. The differential diagnosis between CBD and PSP, both classified as atypical parkinsonisms, remains clinically challenging due to overlapping phenotypic presentations.
