Introduction
Cerebral Ventricles is an important component in the neurobiology of neurodegenerative diseases. This page provides detailed information about its structure, function, and role in disease processes.
Overview

The cerebral ventricles are a system of interconnected, fluid-filled cavities within the brain that produce, circulate, and absorb cerebrospinal fluid (CSF). The ventricular system comprises four chambers — two lateral ventricles, the third ventricle, and the fourth ventricle — connected by narrow passages and lined by ependymal cells ([Sakka et al., 2011](https://pubmed.ncbi.nlm.nih.gov/21482776/)). In the adult human brain, the ventricles contain approximately 25-30 mL of CSF at any given time, while the total CSF volume (including the subarachnoid space) is approximately 125-150 mL, with the choroid plexus producing roughly 500 mL per day ([Johanson et al., 2008](https://pubmed.ncbi.nlm.nih.gov/18456369/)). Ventricular enlargement (ventriculomegaly) is one of the most consistent and readily measurable neuroimaging findings in neurodegenerative disease. In Alzheimer's disease, the rate of ventricular expansion is a sensitive biomarker of disease progression and treatment response, often outperforming hippocampal volumetry in clinical trials ([Nestor et al., 2008](https://pubmed.ncbi.nlm.nih.gov/18687732/)). Disproportionate ventricular enlargement relative to cortical atrophy is the hallmark of normal pressure hydrocephalus, a treatable cause of dementia and gait disturbance ([Relkin et al., 2005](https://pubmed.ncbi.nlm.nih.gov/16186530/)). Anatomy and Structure
Lateral Ventricles
The lateral ventricles are the largest of the four ventricles, one situated within each cerebral hemisphere. Each lateral ventricle is a C-shaped cavity with a capacity of approximately 7-10 mL, consisting of five parts ([Rhoton, 2002](https://pubmed.ncbi.nlm.nih.gov/12234448/)): [@brinker2014]

- Frontal (anterior) horn: Located in the frontal lobe, bounded medially by the septum pellucidum and laterally by the head of the caudate nucleus. The anterior horn contains no choroid plexus
- Body (central part): Extends posteriorly along the parietal lobe, roofed by the corpus callosum and floored by the thalamus and body of the caudate nucleus. The choroid plexus is attached along the choroidal fissure
- Atrium (trigone): The junction where the body, occipital horn, and temporal horn converge. Contains a prominent tuft of choroid plexus (the glomus)
- Occipital (posterior) horn: Extends into the occipital lobe; highly variable in size between individuals and hemispheres
- Temporal (inferior) horn: Curves anteriorly and inferiorly into the temporal lobe. Its floor contains the hippocampus and its roof contains the tail of the caudate nucleus. The choroid plexus extends along the choroidal fissure

The lateral ventricles communicate with the third ventricle through the interventricular foramina (of Monro), located at the anterior end of the body.

Third Ventricle
The third ventricle is a narrow, slit-like midline cavity within the diencephalon, bounded by the thalamus on each side and tightly coupled to hypothalamic neuroendocrine signaling.[@brinker2014][@cinalli2011]
- Roof: Formed by the tela choroidea and choroid plexus, beneath the fornix and corpus callosum
- Floor: Formed by the hypothalamus, including the optic chiasm, tuber cinereum, infundibulum, and mammillary bodies
- Anterior wall: Contains the lamina terminalis and anterior commissure
- Posterior wall: Contains the pineal recess and posterior commissure
- Massa intermedia (interthalamic adhesion): A bridge of gray matter crossing the third ventricle between the two thalami, present in approximately 70% of individuals
The third ventricle connects to the fourth ventricle via the cerebral aqueduct (of Sylvius), a narrow channel passing through the midbrain.
Cerebral Aqueduct
The cerebral aqueduct is the narrowest part of the ventricular system (approximately 1-2 mm in diameter), passing through the midbrain between the tectum (superior colliculus and inferior colliculus) dorsally and the tegmentum ventrally. Its narrow caliber makes it the most common site of ventricular obstruction, leading to non-communicating hydrocephalus ([Cinalli et al., 2004](https://pubmed.ncbi.nlm.nih.gov/15185136/)). The aqueduct is surrounded by the periaqueductal gray, a key structure for pain modulation and autonomic control.
Fourth Ventricle
The fourth ventricle is a tent-shaped cavity located in the posterior fossa, between the brainstem anteriorly and the cerebellum posteriorly.[@nakada2016][@brinker2014]:
- Floor (rhomboid fossa): Formed by the dorsal surfaces of the pons and medulla, containing the nuclei of cranial nerves V through XII
- Roof: Formed by the superior and inferior medullary vela and the tela choroidea
- Lateral recesses: Extend laterally toward the cerebellopontine angle
- Three apertures allow CSF to exit into the subarachnoid space: the median aperture (foramen of Magendie) and two lateral apertures (foramina of Luschka)
The choroid plexus of the fourth ventricle is located in the roof, continuous with the tela choroidea.
Cerebrospinal Fluid Production and Circulation
Choroid Plexus
The choroid plexus is the principal source of CSF production, responsible for approximately 60-70% of total CSF volume. It consists of a highly vascular stroma covered by a single layer of modified ependymal (choroidal epithelial) cells connected by tight junctions that form the blood-CSF barrier ([Lun et al., 2015](https://pubmed.ncbi.nlm.nih.gov/26404473/)). Choroid plexus epithelial cells actively secrete CSF through a process involving:
- Na+/K+ ATPase on the apical membrane
- Carbonic anhydrase-dependent bicarbonate secretion
- Aquaporin-1 (AQP1) water channels
- Chloride channels and transporters
Beyond CSF production, the choroid plexus serves as an immunological sentinel, producing cytokines, growth factors, and transport proteins. It expresses transthyretin, clusterin, and other chaperone proteins that may help clear amyloid-beta/proteins/amyloid from the CNS ([Crossgrove et al., 2005](https://pubmed.ncbi.nlm.nih.gov/16036530/)).
CSF Circulation Pathway
CSF follows a well-defined circulatory route ([Sakka et al., 2011](https://pubmed.ncbi.nlm.nih.gov/21482776/)):
Produced primarily in the lateral ventricles by the choroid plexus
Flows through the interventricular foramina (of Monro) into the third ventricle
Passes through the cerebral aqueduct (of Sylvius) into the fourth ventricle
Exits the fourth ventricle through the median and lateral apertures into the subarachnoid space
Circulates over the surface of the brain and spinal cord
Absorbed primarily by arachnoid granulations (Pacchionian granulations) into the dural venous sinusesRecent research has identified the glymphatic system as an additional CSF-mediated clearance pathway. Driven by arterial pulsation and aquaporin-4 (AQP4) water channels on astrocytic endfeet, the glymphatic system facilitates the exchange of CSF with interstitial fluid along perivascular spaces, clearing metabolic waste including Amyloid-Beta/proteins/amyloid and tau/proteins/tau during sleep ([Iliff et al., 2012](https://pubmed.ncbi.nlm.nih.gov/22896675/); [Xie et al., 2013](https://pubmed.ncbi.nlm.nih.gov/24136970/)).
CSF Functions
CSF serves several critical functions:
- Mechanical protection: Buoyancy reduces the effective brain weight from approximately 1,400 g to approximately 25 g, cushioning against trauma
- Chemical homeostasis: Maintains stable ionic composition (low K+, low protein) for optimal neuronal function
- Waste clearance: Removes metabolic byproducts including Amyloid-Beta/proteins/amyloid, [tau/proteins/tau, and lactate
- Nutrient delivery: Transports vitamins, hormones, growth factors, and neuropeptides
- Immune surveillance: Contains immune cells and immunoglobulins that protect against CNS infection
Ventricular Changes in Neurodegenerative Disease
Alzheimer's Disease
Ventricular enlargement is one of the most robust structural imaging biomarkers of Alzheimer's disease progression. Lateral ventricular volume increases at approximately 1-2 mL/year in healthy aging, but accelerates to 3-8 mL/year in AD, with the rate correlating with cognitive decline ([Nestor et al., 2008](https://pubmed.ncbi.nlm.nih.gov/18687732/); [Jack et al., 2004](https://pubmed.ncbi.nlm.nih.gov/15207764/)).
Key findings include:
- Temporal horn enlargement: Dilatation of the temporal horns reflects hippocampal and medial temporal lobe atrophy and is an early marker of AD, sometimes preceding clinical symptoms ([Ferrarini et al., 2008](https://pubmed.ncbi.nlm.nih.gov/17986024/))
- Treatment response biomarker: In clinical trials of anti-amyloid therapies, ventricular volume change has served as a structural endpoint, with reduced expansion rates indicating potential treatment benefit
- Boundary shift integral: Automated measurement of ventricular boundary expansion provides sensitive detection of brain atrophy at rates as low as 0.5% per year
- CSF biomarkers: Ventricular (and lumbar) CSF shows decreased Amyloid-Beta/proteins/amyloid 42 and increased phosphorylated [tau in AD, forming the basis of the ATN biomarker framework ([Jack et al., 2018](https://pubmed.ncbi.nlm.nih.gov/29653606/))
Choroid plexus dysfunction in AD includes reduced CSF production, impaired clearance of [Amyloid-Beta/proteins/amyloid, and increased blood-CSF barrier permeability ([Serot et al., 2000](https://pubmed.ncbi.nlm.nih.gov/11073086/)).
Normal Pressure Hydrocephalus
Normal pressure hydrocephalus (NPH) is characterized by the clinical triad of gait disturbance, urinary incontinence, and cognitive impairment, combined with ventriculomegaly out of proportion to cortical atrophy ([Relkin et al., 2005](https://pubmed.ncbi.nlm.nih.gov/16186530/)). NPH is a critically important diagnosis because it is one of the few reversible causes of dementia, treatable by CSF shunting.
Imaging features distinguishing NPH from neurodegenerative ventriculomegaly include:
- Evans index > 0.3: Ratio of maximum frontal horn width to maximum internal skull diameter
- Callosal angle < 90 degrees: Measured on coronal MRI at the level of the posterior commissure
- Disproportionately enlarged subarachnoid space hydrocephalus (DESH): Tight high-convexity sulci with dilated Sylvian fissures
- Periventricular hyperintensities: Reflecting transependymal CSF migration
- Flow void sign: Hyperdynamic CSF flow in the cerebral aqueduct on MRI
Huntington's Disease
In Huntington's disease, caudate nucleus atrophy produces characteristic enlargement of the frontal horns ("box-shaped" ventricles), visible even in presymptomatic HTT gene carriers years before clinical onset ([Aylward et al., 2004](https://pubmed.ncbi.nlm.nih.gov/15024700/)). The rate of frontal horn enlargement correlates with CAG repeat length and serves as a progression biomarker in clinical trials.
Frontotemporal Dementia
frontotemporal dementia produces asymmetric ventricular enlargement reflecting the pattern of cortical atrophy:
- Behavioral variant FTD: Enlargement of the frontal horns, often asymmetric
- Semantic dementia: Temporal horn enlargement, typically left-predominant
- Primary progressive aphasia: Left-sided frontal and temporal horn enlargement
The C9orf72 repeat expansion, the most common genetic cause of FTD and ALS, produces symmetric, predominantly posterior-predominant ventricular enlargement that may mimic normal pressure hydrocephalus ([Whitwell et al., 2012](https://pubmed.ncbi.nlm.nih.gov/22710621/)).
Parkinson's Disease and Lewy Body Dementia
Ventricular enlargement in Parkinson's disease is modest compared to AD and correlates primarily with cognitive decline and progression to Parkinson's Disease dementia. In Lewy body dementia, posterior-predominant ventricular enlargement reflects occipitoparietal atrophy, correlating with the visual hallucinations and visuospatial deficits characteristic of the disease ([Burton et al., 2004](https://pubmed.ncbi.nlm.nih.gov/15534243/)).
Multiple Sclerosis
In multiple sclerosis, ventricular enlargement results from periventricular white matter loss. The third ventricle width has been proposed as a simple and reliable measure of central brain atrophy that correlates with disability progression ([Benedict et al., 2006](https://pubmed.ncbi.nlm.nih.gov/16894899/)).
CSF as a Diagnostic Window
Ventricular cerebrospinal fluid (CSF) provides direct biochemical readouts of central nervous system pathology and is a core compartment for fluid biomarker development in neurodegenerative disease.[@lehtinen2011][@blennow2015]
Core AD Biomarkers
- Amyloid-Beta 42 (Abeta42): Decreased in AD CSF due to plaque sequestration and altered clearance kinetics[@blennow2015]
- Total tau (t-tau: Elevated in AD and related disorders as a marker of neuronal injury[@blennow2015]
- Phosphorylated tau (p-tau181, p-tau217): More AD-specific than total tau and increasingly used for biological diagnosis and trial enrichment[@blennow2015]
Emerging Biomarkers
- Neurofilament light chain: Reflects large-caliber axonal injury across multiple neurodegenerative conditions
- GFAP: Tracks astroglial activation and reactive gliosis
- alpha-synuclein seed amplification assays: Detect misfolded alpha-synuclein species in synucleinopathies
- TDP-43 assays: Under active development for ALS and Frontotemporal Dementia (FTD)
Clinical Translation Considerations
CSF biomarkers are most informative when interpreted as multimarker panels (amyloid, tau, glial and axonal injury markers) rather than isolated tests.
Pre-analytical handling, assay platform harmonization, and cutpoint calibration remain major determinants of cross-cohort reproducibility and real-world deployment.[@blennow2015]
Development and Aging
Embryonic Development
The ventricular system develops from the central canal of the neural tube. During early embryogenesis, the neural tube expands to form three primary brain vesicles (prosencephalon, mesencephalon, rhombencephalon), whose lumina become the ventricles. The lateral ventricles arise from the telencephalic vesicles, while the third ventricle derives from the diencephalic vesicle.
Normal Aging
Ventricular volume increases linearly with age, approximately doubling between ages 20 and 80 ([Scahill et al., 2003](https://pubmed.ncbi.nlm.nih.gov/12815585/)). This normal age-related ventriculomegaly results from gradual brain parenchymal loss (approximately 0.2-0.5% brain volume per year after age 60) and is distinct from the accelerated enlargement seen in neurodegenerative disease. Choroid plexus calcification and fibrosis increase with age, potentially reducing CSF production and waste clearance capacity.
