Overview

SERPINA1 encodes alpha-1 antitrypsin (A1AT), a member of the serpin superfamily of serine protease inhibitors. While classically studied in the context of lung emphysema and liver disease due to deficiency states, emerging evidence positions SERPINA1 as a significant player in neurodegenerative disease pathogenesis. Alpha-1 antitrypsin functions as an acute-phase protein with broad anti-inflammatory properties, including inhibition of neutrophil elastase, cathepsin G, and proteinase-3. In the central nervous system, A1AT is produced by astrocytes and microglia, where it modulates neuroinflammation, oxidative stress, and proteostasis—all key processes in Alzheimer's disease (AD), Parkinson's disease (PD), and amyotrophic lateral sclerosis (ALS) pathogenesis.
