CADASIL (Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy) is a hereditary small vessel disease caused by NOTCH3 mutations. Treatment focuses on stroke prevention, symptom management, and lifestyle modifications, as no disease-modifying therapy exists currently. [@cadasil2023]

Aspirin and Antiplatelet Agents

Antiplatelet therapy is a cornerstone of CADASIL management to reduce ischemic stroke risk:

  • Low-dose aspirin (81-100 mg daily) is commonly prescribed for primary and secondary stroke prevention [@antiplatelet2013](https://pubmed.ncbi.nlm.nih.gov/23435542/)
  • Clopidogrel may be used in patients who cannot tolerate aspirin
  • Dipyridamole combined with aspirin has been studied but evidence specific to CADASIL is limited
  • Dual antiplatelet therapy (aspirin + clopidogrel) may be considered after an acute ischemic event, but bleeding risk must be carefully evaluated
