Despite decades of research, no therapy has achieved definitive neuroprotection in a major neurodegenerative disease clinical trial. However, advances in understanding the molecular mechanisms of neuronal death — including mitochondrial dysfunction, oxidative stress, excitotoxicity, neuroinflammation, and protein aggregation — have yielded an expanding pipeline of neuroprotective candidates. Mitochondrial Dysfunction and Energy Failure
Mitochondrial dysfunction is a hallmark of virtually all neurodegenerative diseases including [Alzheimer's disease](/diseases/alzheimers-disease), [Parkinson's disease](/diseases/parkinsons-disease), and [ALS](/diseases/amyotrophic-lateral-sclerosis). Impaired oxidative phosphorylation, excessive reactive oxygen species (ROS production, defective [mitophagy](/mechanisms/mitophagy), and disrupted calcium buffering contribute to neuronal energy failure. Oxidative Stress

Neuroinflammation
