Hereditary spastic paraplegia (HSP), also known as familial spastic paraparesis (FSP) or Strümpell-Lorrain disease, is a clinically and genetically heterogeneous group of inherited neurodegenerative disorders characterized by progressive spasticity and weakness of the lower limbs due to length-dependent degeneration of the corticospinal tract axons. The pathological hallmark is a "dying-back" axonopathy: the longest motor axons in the corticospinal tract (those projecting to the lumbar spinal cord) degenerate from their distal ends, producing the characteristic pattern of lower extremity spasticity while initially sparing shorter axons.
