• Intermediate-risk phenotypes (30-70% associated with cancer): brainstem encephalitis, stiff-person spectrum disorder, myelitis, Motor Neuron Disease-like presentations, autoimmune autonomic ganglionopathy

• High-risk antibodies (>70% cancer association): anti-Hu (ANNA-1), anti-Yo (PCA-1), anti-CV2/CRMP5, anti-Ri (ANNA-2), anti-amphiphysin, anti-Ma2 (in males >50), anti-PCA-Tr/DNER, anti-SOX1
• Intermediate-risk antibodies (30-70% cancer association): anti-CASPR2, anti-AMPAR, anti-DPPX, anti-IgLON5
• Lower-risk antibodies: anti-NMDAR (except in teratoma), anti-LGI1, anti-GABA-B

Major Syndromes

Paraneoplastic Cerebellar Degeneration (PCD)

PCD is one of the most common and devastating PNS, characterized by subacute pancerebellar dysfunction with severe truncal and appendicular ataxia, dysarthria, and nystagmus developing over weeks to months. The cerebellum is particularly vulnerable due to the high concentration of onconeural antigens in Purkinje cells. [@neuroinflammation]

• Anti-Yo (PCA-1): Most common; associated with ovarian cancer and breast cancer. Targets cerebellar degeneration-related protein 2 (CDR2) and CDR2L in Purkinje cells
• Anti-Tr/DNER: Associated with Hodgkin lymphoma. Targets Delta/Notch-like EGF-related receptor on Purkinje cells
• Anti-Hu (ANNA-1): Associated with small cell lung cancer (SCLC). Usually presents as part of multifocal encephalomyelitis with cerebellar involvement
• Anti-VGCC: Associated with SCLC, often co-occurring with Lambert-Eaton syndrome
• Anti-mGluR1: Associated with Hodgkin lymphoma

Paraneoplastic Limbic Encephalitis

Limbic encephalitis presents with subacute onset of short-term memory loss, seizures, psychiatric symptoms (anxiety, depression, psychosis), and confusion due to inflammatory involvement of the [hippocampus](/brain-regions/hippocampus), amygdala, and medial temporal lobes. [@alzheimers]

• Anti-Hu: SCLC (most common paraneoplastic cause of limbic encephalitis)
• Anti-Ma2: Testicular germ cell tumors (young males), lung cancer (older patients)
• Anti-AMPAR: Breast, lung, thymic tumors
• Anti-GABA-B: SCLC
• Anti-CASPR2: Thymoma (also occurs without cancer)

Paraneoplastic Encephalomyelitis

The most extensive paraneoplastic syndrome, encephalomyelitis involves multiple levels of the nervous system simultaneously, including cerebral [cortex](/brain-regions/cortex), limbic system, brainstem, spinal cord, dorsal root ganglia, and autonomic nervous system. Most commonly associated with anti-Hu antibodies and SCLC. [@multiplesclerosis]

Clinical features include a combination of:

• Cognitive decline and personality changes (cortical involvement)
• Limbic encephalitis symptoms
• Brainstem dysfunction (cranial neuropathies, central hypoventilation)
• Myelopathy
• Sensory neuronopathy
• Autonomic failure (gastroparesis, orthostatic hypotension, urinary retention)

Sensory Neuronopathy (Denny-Brown Syndrome)

Subacute sensory neuronopathy affects dorsal root ganglia, producing asymmetric, painful paresthesias and progressive proprioceptive loss leading to sensory ataxia. Most commonly associated with anti-Hu antibodies and SCLC. Pathology shows destruction of dorsal root ganglion neurons with T-cell infiltration.

Lambert-Eaton Myasthenic Syndrome (LEMS)

LEMS is a disorder of the neuromuscular junction caused by antibodies against presynaptic P/Q-type voltage-gated calcium channels (VGCC). Approximately 50-60% of LEMS cases are paraneoplastic, predominantly associated with SCLC. Clinical features include proximal weakness (legs > arms), autonomic dysfunction (dry mouth, constipation), and hyporeflexia with post-exercise facilitation.

Stiff-Person Spectrum Disorders

While most cases of stiff-person syndrome are associated with anti-GAD65 antibodies and are non-paraneoplastic, a subset with anti-amphiphysin antibodies is strongly associated with breast cancer and SCLC. Paraneoplastic stiff-person syndrome presents with progressive rigidity, painful spasms, and hyperekplexia.

Opsoclonus-Myoclonus Syndrome (OMS)

OMS presents with chaotic, involuntary eye movements (opsoclonus), myoclonus, and ataxia. In children, it is strongly associated with neuroblastoma. In adults, OMS is associated with breast cancer, ovarian cancer, and SCLC. Anti-Ri (ANNA-2) antibodies are found in some adult cases.

Immunopathogenesis

Antibody-Mediated vs. T-Cell-Mediated Mechanisms

PNS are classified into two major immunopathogenic categories based on the target antigen location:

Intracellular antigen targets (T-cell mediated):
Antibodies against intracellular antigens (Hu, [Yo/CDR2](/proteins/cdr2-protein), Ri, CV2, Ma2, amphiphysin) are markers of the immune response but are not directly pathogenic. Neuronal damage is primarily mediated by CD8+ cytotoxic T lymphocytes that recognize tumor-derived antigens cross-presented on neuronal MHC class I molecules. This mechanism leads to irreversible neuronal destruction, explaining the poor response to immunotherapy in these syndromes.

Evidence supporting T-cell-mediated pathogenesis:

• CD8+ T-cell infiltrates in affected nervous system tissue
• Clonal T-cell expansion targeting specific epitopes
• Persistence of antibodies after neuronal loss
• Poor response to plasma exchange and B-cell depletion

Immune Surveillance and Tumor Control

A remarkable aspect of PNS is that patients often have better cancer outcomes than non-PNS patients with the same tumor type. The anti-tumor immune response that produces PNS also provides effective tumor surveillance. SCLC patients with anti-Hu antibodies, for example, tend to have limited-stage disease and longer survival, as the immune response keeps the tumor in check while unfortunately cross-reacting with the nervous system.

Role of neuroinflammation

PNS involve activation of innate and adaptive immune pathways within the nervous system. Microglial activation, perivascular lymphocytic infiltrates, and complement deposition are common pathological features. The inflammatory microenvironment both contributes to neuronal dysfunction and represents a potential therapeutic target.

Diagnosis

Clinical Approach

If initial cancer screening is negative, repeat screening at 3-6 month intervals for at least 2 years, as the cancer may be occult at the time of neurological presentation.

Differential Diagnosis

PNS must be distinguished from:

• Viral and autoimmune encephalitis
• Autoimmune encephalitis (non-paraneoplastic)
• Neurodegenerative diseases (Alzheimer's Disease, FTD, Prion Disease)
• CNS vasculitis
• Metabolic encephalopathies
• Direct tumor effects (metastases, leptomeningeal disease)
• Treatment toxicity (chemotherapy, radiation, immunotherapy)

Treatment

Principles of Management

Treatment of PNS rests on three pillars:

Immunotherapeutic Approaches

First-line therapy:

• High-dose intravenous corticosteroids (methylprednisolone 1g/day × 5 days)
• Intravenous immunoglobulin (IVIg; 0.4 g/kg/day × 5 days)
• Plasma exchange (5-7 sessions)

• Rituximab (anti-CD20; particularly effective in cell-surface antibody syndromes)
• Cyclophosphamide (especially for T-cell-mediated syndromes)
• Tocilizumab (anti-IL-6 receptor)
• Bortezomib (proteasome inhibitor targeting plasma cells)

• Cell-surface antibody syndromes: 60-80% show meaningful improvement with immunotherapy
• Intracellular antibody syndromes: <30% show significant improvement; early treatment provides the best chance of stabilization rather than recovery

Prognosis

Prognosis varies markedly by syndrome and antibody type:

• Best outcomes: LEMS (with cancer treatment), anti-NMDAR encephalitis (with teratoma removal), anti-CASPR2/LGI1 limbic encephalitis
• Moderate outcomes: Anti-Ma2 encephalitis (with testicular tumor treatment), OMS
• Poor outcomes: PCD (especially anti-Yo), anti-Hu encephalomyelitis, sensory neuronopathy

Relationship to Neurodegenerative Disease
