The disease was first described pathologically in the 1960s, but its genetic basis remained elusive until 2019, when abnormal GGC trinucleotide repeat expansions in the NOTCH2NLC gene were identified as the causative mutation in both familial and sporadic cases. NIID is now recognized as a member of the expanding family of repeat expansion disorders, with remarkable clinical heterogeneity ranging from dementia and cerebellar ataxia to parkinsonism, peripheral neuropathy, and episodic encephalopathy .
