Transcranial magnetic stimulation (TMS) represents an alternative non-invasive approach for therapeutic delivery to the central nervous system through targeted modulation of neuronal activity-dependent vascular permeability. Unlike ultrasound-mediated mechanical disruption, TMS exploits the intrinsic coupling between neuronal firing patterns and cerebrovascular dynamics to enhance therapeutic penetration. The mechanism centers on TMS-induced depolarization of cortical pyramidal neurons, which triggers synchronized calcium influx through voltage-gated calcium channels (VGCCs), particularly L-type Cav1.2 channels. This calcium surge activates neuronal nitric oxide synthase (nNOS), leading to rapid nitric oxide (NO) production and subsequent activation of soluble guanylate cyclase in adjacent brain microvascular endothelial cells.
