Overview

REM Sleep Behavior Disorder (RBD) is a parasomnia characterized by the loss of normal skeletal muscle atonia during rapid eye movement (REM) sleep, resulting in dream-enactment behaviors that can range from simple limb movements to violent thrashing, punching, and kicking. First described by Carlos Schenck and Mark Mahowald in 1986, RBD has emerged as one of the most important prodromal markers of alpha-synucleinopathies, including parkinsons, lewy-body-dementia, and msa. The prevalence of RBD in the general population is estimated at 0.5–2%, with higher rates in men and older adults. However, the clinical significance of RBD extends far beyond its sleep-related symptoms: longitudinal studies have established that more than 80% of individuals with isolated RBD (iRBD)—those without an overt neurodegenerative diagnosis—will eventually develop a full synucleinopathy, with annual phenoconversion rates of 6–8%.
