What is RT-QuIC?

RT-QuIC is an ultra-sensitive assay that detects minute amounts of misfolded α-synuclein seeds by their ability to seed the aggregation of recombinant α-syn in vitro[@fairfoul2016]. The assay produces a Thioflavin T fluorescence signal when seeds are present.

Clinical Utility

Sensitivity: 90-95% in PD, 60-70% in DLB[@sano2018]
Specificity: 90-100% vs healthy controls[@sano2018]
Prodromal detection: Can identify at-risk individuals

Patient Selection

RT-QuIC positive in CSF
Clinical diagnosis of PD, DLB, or MSA
Disease duration <5 years
Mild to moderate symptoms (H&Y 1-3)
