🧪 Overview

Mechanistic Overview

The Glial Ketone Metabolic Shunt Hypothesis starts from the claim that modulating HMGCS2 within the disease context of neurodegeneration can redirect a disease-relevant process. The original description reads: "The Glial Ketone Metabolic Shunt Hypothesis proposes that reactive astrocytes in neurodegenerative disease aberrantly upregulate ketone body synthesis (ketogenesis), creating a metabolic steal syndrome that depletes shared glucose and lipid substrates from neurons while producing ketone bodies that failing neurons cannot efficiently metabolize — a paradoxical "rescue attempt" that worsens energy crisis. Background and Rationale Brain energy metabolism represents one of the most tightly regulated biological systems, with astrocytes and neurons maintaining exquisite metabolic coupling to support the enormous energy demands of neural computation. The human brain consumes approximately 20% of total body glucose despite representing only 2% of body weight, highlighting the critical importance of efficient energy substrate utilization.

Abstract
Histone-modifying enzymes are implicated in the control of diverse DNA-templated processes including gene expression. Here, we outline historical and current thinking regarding the functions of histone modifications and their associated enzymes. One current viewpoint, based largely on correlative evidence, posits that histone modifications are instructive for transcriptional regulation and represent an epigenetic 'code'. Recent studies have challenged this model and suggest that histone marks pr

Intravenous drug use (IDU) poses a high risk of serious complications such as infective endocarditis (IE), which carries high morbidity and mortality rates. Mycotic pulmonary artery aneurysms (MPAA) are rarely associated with right-sided IE, especially in the setting of IDU. It is a potentially fatal complication as it can lead to severe hemorrhage if the aneurysm ruptures. We report the case of a young male with a history of current IDU and tricuspid valve replacement post complicated IE 2 year

Abstract
Ketogenesis, a mitochondrial metabolic pathway, occurs primarily in liver, but kidney, colon and retina are also capable of this pathway. It is activated during fasting and exercise, by "keto" diets, and in diabetes as well as during therapy with SGLT2 inhibitors. The principal ketone body is β-hydroxybutyrate, a widely recognized alternative energy source for extrahepatic tissues (brain, heart, muscle, and kidney) when blood glucose is sparse or when glucose transport/metabolism is impaired.
