Rationale
Background
Alzheimer's disease is characterized by progressive accumulation of amyloid-beta plaques and tau neurofibrillary tangles, but emerging evidence points to network dysfunction as an early driver of pathology. Resting-state fMRI studies consistently show increased functional connectivity in early AD, particularly in default mode network regions. This hyperconnectivity may facilitate spread of pathological tau through connected brain networks.
The Gut-Brain-Vagus Hypothesis
Recent research from Nature (2026) demonstrates that age-related memory decline is driven by breakdown in gut-to-brain signaling via the vagus nerve. This pathway involves:
- Parabacteroides goldsteinii: Gut bacteria that increase with age
- Metabolite blockade: These bacteria release metabolites that block vagal signaling
- Myeloid inflammation: Resulting peripheral inflammation affects brain
- Cognitive decline: Memory impairment through this pathway
The vagus nerve projects to brainstem nuclei that modulate network excitability. Vagus nerve stimulation (VNS) in older mice restored memory to young levels. This suggests that network-targeted neuromodulation could reverse connectivity abnormalities.
tACS Mechanism
tACS delivers alternating currents at specific frequencies (typically 10-40 Hz) to entrain neural oscillations. In AD:
- Gamma-band tACS (40 Hz): May enhance gamma oscillations impaired in AD
- Targeted connectivity modulation: Frequency-tuned to normalize hyperconnected networks
- Tau reduction hypothesis: Normalized activity reduces activity-dependent tau spread

Mechanistic Hypotheses
Hypothesis 1: Connectivity Normalization
tACS at individualized frequencies will reduce abnormal hyperconnectivity in early AD networks, particularly in the default mode network and salience network.
Hypothesis 2: Tau Spread Reduction
By normalizing network activity, tACS will reduce activity-dependent tau propagation along connected neural pathways.
Hypothesis 3: Cognitive Improvement
Connectivity normalization + tau reduction will translate to preserved or improved cognitive function.
